Public libraries are no longer a distribution center for state tax forms and booklets. If you filed a paper form last year, you will receive a paper form in the mail this year. If you need a booklet, call Oregon Department of Revenue at 503.378.4988 or 1.800.356.4222 or e-mail

State tax forms can be downloaded online at the Oregon Department of Revenue. Scappoose Public Library staff are happy to help you find and print Oregon tax forms.*
